What does it mean in the Docs where they say:
“If you do not use service accounts, the following is true”, then different things are “marked as” user mailboxes, shared mailboxes etc.
What does it mean “marked as”? Where is it marked? For what purpose is it marked?

You are correct it is not required. However, it does affect the ability to discover specific mailbox types and those are listed/labeled as the type noted in docs when you see them in the GUI. This is noted in the mailbox types column in CommandCenter see screenshot below.
It is always recommended to use the express setup, which requires to enter an account to create the apps, etc then you can decide if you want to leave the account or not. Its only used for discovery purposes not backup/data movement that is done via the apps.

No it does not it's still one user mailbox.
